CAUSES AND TREATMENT OF MIGRAINES
The causes of migraines are not 100% understood. However, the triggers for migraines include taking medication, drinking alcohol – especially red wine – too much caffeine and stress. Sometimes, however, subtle stimuli such as bright light, lack of sleep or strong odors can cause the severe and all-consuming headache.
On a physiological-anatomical level, the chronic migraine is attributed to a cortical hyperexcitability, i.e. affecting the cerebral cortex, which occurs as a result of increased sensitivity to pain stimuli in the central nervous system (CNS).
There are few effective drugs on the market that reliably help with migraines.. It is also often important to take them at the right time – if they are taken a little too late, they will no longer help with the headache. Often all that remains is retreating into a dark room and waiting until the attack of pain slowly subsides.
BOTOX® FOR CHRONIC MIGRAINES
Botulinum neurotoxin A, better known as Botox®, is particularly effective in the treatment of chronic migraines and was therefore approved for their treatment in Germany in 2011.
Botulinum toxin A is a so-called “neurotoxin”. It inhibits the nerve endingswhose activity is responsible for muscle tension or wrinkles through the release of the neurotransmitter acetylcholine. The doses are so low that, contrary to what the name suggests, it is not “toxic”, i.e. poisonous, but merely has a relaxing effect on the treated area, resulting in a relaxation of the muscles. The wrinkle-reducing effect of “Botox” has long been used in aesthetic medicine.
Scientific studies have shown that botulinum toxin type A, in addition to the aforementioned inhibition of nerve cells, prevents the release of certain proteins that are involved in the “development and maintenance of pain and inflammation.”*
Treatment of the head muscles with botulinum toxin type A canprevent the physiological cause of migraine, the central hyperexcitabilitythat leads to the massive pain. Botulinum toxin A therapy is therefore used in particular as a preventive therapy.
The botulinum toxin A is injected into the neck area, in the temporal region and into the muscles that form the frown lines (M. procerus and M. corrugartor supercilii) – depending on where the pain begins. If the pain is particularly intense there, the dose in this area must be adjusted.
The use of Botox® as an anti-migraine agent is well established, safe and the compatibility is excellent.. The muscle-relaxing and pain-modulating effects in the CNS can bring about a rapid improvement in migraines. Since Botox® acts locally, it cannot cause systemic side effects or central nervous effects. The side effects are also minor compared to the known migraine prophylactics.
